Select New Patient Registration Tab, review wlecome message and click next. After submtting your online registration the office will contact you and schedule an appointment with the bariatric coordinator.
If you do not have internet access give us a call and set up an appointment with our bariatric coordinator. We will answer all of your questions and provide you with additional in-depth information. The more knowledge you have about weight loss surgery, the easier it will be for you to make an informed decision.
Please note, we currently do not accept Medicare Insurance for weight loss surgery.
